as ISATU JALLOH of NO.5 Barrie Drive Allen Town, FREETOWN, in the Eastern Urban area of the republic of sierra Leone aforesaid a citizen of the Republic of Sierra Leone by birth.
I on my behalf, wholly renounce relinquish and abandon the use of the NAME of ISATU and in place thereof I assume from today the NAME of HARISATU and that I shall from today be called Known and distinguished not by my former NAME of ISATU but by my present forename and surname of HARISATU JALLOH.
I shall at all times from today in all records, deeds and writing and in all proceeding, dealings and transactions, private as well as public, and on all occasions use and sign the new name of HARISATU JALLOH.
