BY THIS DEED which is intended to be registered in the office of the Administrator and Registrar- General, Roxy Building, Freetown in the Western Area of the Republic of Sierra Leone, I the undersigned UMARO SESAY of No4 Turay Drive, Kossoh Town, Jui, Freetown aforesaid now lately called UMAR BAH a citizen of the Republic of Sierra Leone do hereby.
For and on behalf of myself and remoter issue wholly renounce, relinquish and abandon the use of my former first name UMARO SESAY and in place thereof do assume from date hereof the name of UMAR BAH and so that I may remoter issue hereafter be called, known and distinguished not by my former name UMARO SESAY but by the name UMAR BAH for the purpose of evidencing such my determination declare that I shall at all times hereafter in all records Deeds and writings in all proceedings dealing and transactions as well private as public and upon all occasion whatsoever use and sign the name of UMAR BAH as my name in place of and in substitution for my former name of UMARO SESAY Expressly authorized and request all persons at all-time hereafter to designate and address me and remoter issue by such assumed name of UMAR BAH accordingly.
