By this DEED I the undersigned IBRAHIM KEMOH SANDI of No. 7 Sobu Sogbandi Street Yemoh Town Section, Bo City, Kakua Chiefdom, Bo District in Southern Province of the Republic of Sierra Leone, a natural born Sierra Leonean subject do hereby absolutely renounce and abandon the use of my former name IBRAHIM I. KAMARA and in lieu thereof assume as from the date hereof the name of IBRAHIM KEMOH SANDI.
And in PURSUANCE of such name as aforesaid, I hereby declare that I shall at all times hereafter in all records, DEED, instruments in writing and in all actions and proceedings and in all dealings and transactions and upon all occasions whatsoever use and sign the said name of IBRAHIM KEMOH SANDI as my name in lieu of the said name of IBRAHIM I. KAMARA so renounced as aforesaid.
AND I HEREBY authorize and request all persons to designate and address me by such assumed name IBRAHIM KEMOH SANDI.
IN WITNESS WHERE i have hereunto signed my assumed name of IBRAHIM KEMOH SANDI for my relinquished name of IBRAHIM I. KAMARA and have set my seal this day of 27th day of November 2023.
